What should I do prior to my visit?

Complete your new patient health history form and make sure that it gets back to us prior to your scheduled appointment to avoid being rescheduled. If you have had any testing done prior to your appointment that pertains to the reason you are coming to visit us please be sure all reports are sent or that you have them to bring with you. We do request that any CT scans or MRIs have the accompanying images. You can get a CD with the images from the facility that did the scans for you. If you are coming for a second opinion or if you are changing doctors, please be sure all records are sent to us or bring them with you.
